EFEK RASIO KOLESTEROL TOTAL/KOLESTEROL HIGH DENSITY LIPOPROTEIN (HDL) TERHADAP NILAI ANKLE BRACHIAL INDEX (ABI) PADA POPULASI DENGAN RISIKO PENYAKIT ARTERI PERIFER (PAP)

EFEK RASIO KOLESTEROL TOTAL/KOLESTEROL HIGH DENSITY LIPOPROTEIN (HDL) TERHADAP NILAI ANKLE BRACHIAL INDEX (ABI) PADA POPULASI DENGAN RISIKO PENYAKIT ARTERI PERIFER (PAP)

Latar Belakang: Prediktor utama terjadinya penyakit kardiovaskular ialah Penyakit Arteri Perifer (PAP) yang patogenesis utamanya adalah aterosklerosis. Risiko terjadinya aterosklerosis meningkat pada kondisi dimana terjadi peningkatan kadar kolesterol total yang diikuti dengan penurunan kadar High Density Lipoprotein (HDL). Ankle Brachial Index (ABI) merupakan tes skrining non invasif untuk mendeteksi terjadinya PAP.
Tujuan: Untuk mengetahui hubungan rasio kolesterol total/HDL terhadap nilai ABI pada populasi dengan faktor risiko PAP.
Metode: Penelitian ini merupakan penelitian analitik observasional dengan menggunakan metode cross sectional. Pengambilan data penelitian dilakukan di RSUD Provinsi NTB, RS Harapan Keluarga, dan RS Risa Sentra Medika. Kadar kolseterol total dan HDL didapatkan melaui hasil pemeriksaan laboratorium.
Hasil: Total responden adalah 92, 17,4% memiliki nilai kolesterol total abnormal, 44,6% HDL abnormal dan 51,1% rasio kolesterol total/HDL abnormal. Hasil uji chi-square kolesterol total, HDL, dan rasio kolesterol total/HDL dengan nilai ABI (p=0,525), (p=0,397), (p=0,278). Hasil uji Rasio Odds kolesterol total, HDL, dan rasio kolesterol total/HDL dengan nilai ABI (RO=1,465), (RO=1,505), (RO=1,697).
Kesimpulan: Tidak terdapat hubungan yang signifikan antara kadar kolesterol total, HDL, dan rasio kolesterol total/HDL terhadap nilai ABI karena sebagian besar responden merupakan penderita dislipidemia yang telah mengonsumsi obat anti-dislipidemia dan abnormalitas nilai ABI dipengaruhi oleh banyak faktor risiko. Individu dengan kadar kolesterol total, kolesterol HDL, dan rasio kolesterol total/HDL abnormal mempunyai risiko 1,465, 1,505, dan 1,697 kali untuk memiliki nilai ABI abnormal.

EFFECT OF TOTAL CHOLESTEROL/HIGH-DENSITY LIPOPROTEIN CHOLESTEROL (HDL) RATIO ON ANKLE-BRACHIAL INDEX (ABI) VALUE IN POPULATION AT RISK OF THE PERIPHERAL ARTERIAL DISEASE (PAD)

Background: The main predictor of cardiovascular disease is Peripheral Arterial Disease (PAD) which is the main pathogenesis of atherosclerosis. The risk of atherosclerosis increase in circumstances where there is an increase in total cholesterol levels that are followed by decrease levels of High Density Lipoprotein (HDL). Ankle-Brachial Index (ABI) is a non-invasive test to detect the onset of PAD.
Objective: The purpose of this study was to investigate the correlation between total cholesterol/HDL ratios with ABI values in the populations at risk of the PAD.
Methods: The research was an analytic observational using cross-sectional method. Data research was collected in NTB General Hospital, Harapan Keluarga, and Risa Sentra Medika Hospital. Levels of total cholesterol and HDL obtained through laboratory examination findings. Results:The total respondents were 92, 17,4% respondents had abnormal total cholesterol, 44,6% had abnormal HDL and 51,1% had abnormal total cholesterol/HDL ratio. Result of Chi-square test showed the correlation between total cholesterol, HDL, total cholesterol/HDL ratio with ABI value (p=0.525), (p=0.397), (p=0.278). Result of odds ratio test showed total cholesterol, HDL, total cholesterol/HDL ratio with ABI value (OR=1.465), (OR=1.505), (OR=1.697).
Conclusion: There was no significant correlation between total cholesterol, HDL, and total cholesterol/HDL ratio with ABI value because most respondents are people with dyslipidemia who had been taking medicine and abnormal ABI value was influenced by many risk factors. People with abnormal total cholesterol, HDL, and total cholesterol/HDL ratio had the possibility 1,465, 1,505, and 1,697 times to have abnormal ABI value.
